The ELDAC Home Care App has been built with the help of care workers and managers to guide aged care workers on caring for their home care clients at the end of life. It helps care workers to be more and confident and comfortable when having difficult conversations and managing care issues.
The app can guide you to:
- Talk to your clients and their family about planning and preparing for the end of life.
- Share resources about advance care planning and palliative care with your client and their loved ones.
- Notice changes in your client’s health and how to support them as their needs change.
- Care for your client right up to the end of life, including providing palliative care.
- Support your client’s family and carers with feelings of grief and loss.
- Suggests resources for you to share with your clients, carers, and families.
The Home Care App supports you to care for yourself with suggestions of self-care activities you can schedule as part of your daily or weekly routine.
The ELDAC Home Care App has been built by the Technology and Innovation team at Flinders University and is based on practical, evidence-based information. ELDAC (www.eldac.com.au) is a national palliative care project funded by the Australian Government Department of Health and Aged Care.
By downloading and using the ELDAC Home Care App, you will also be invited to participate in an evaluation research study. Participation is voluntary, and you can opt out at any time.
